About the Position

A growing, physician-led medical group dedicated to providing high-quality care in Skilled Nursing, Long-Term Care, Assisted Living, Independent Living, and Rehabilitation settings is seeking a compassionate, motivated Nurse Practitioner to join its team just 15 miles from Boulder, Colorado.

Responsibilities

  • Provide medical care and follow-up for patients in post-acute care settings.
  • Collaborate with physicians, therapists, case managers, and facility staff to coordinate patient care.
  • Complete timely and accurate clinical documentation.
  • Participate in quality improvement initiatives and ensure compliance with regulatory requirements.
  • Travel to assigned Skilled Nursing, Assisted Living, and Independent Living facilities throughout the Denver metro area.

Qualifications

  • Family, Acute Care, or Adult Gerontology Nurse Practitioner Certified
  • Active Colorado NP license, or ability to obtain.
  • 1+ years of Post-acute care, geriatrics, family medicine, internal medicine, or skilled nursing experience preferred
